2011 Women's Hall of Fame
Nomination Form - Part 1

Submit Nomination Forms, Photograph, Biographical Information, Biographical Statements of Qualifications and supporting documents by July 18, 2011.

Selection Criteria

  1. Nominee must be a woman whose life and works have made significant contributions to the improvement and enhancement of life for all women and for all citizens of Okaloosa county.
  2. Nominee's accomplishments may be in one or more fields of art, agriculture, athletics, business, community services, education, environment, government, health, humanities, law, philanthropy, and/or science.
  3. Nominee must be or have been a resident of Okaloosa County; she may be living or deceased.

Criteria for Judging
The commission will assess each nominee on the following criteria:

  • Professional Qualifications
  • Community Service
  • Length of Residence in Okaloosa County
  • Amount of Woman-Specific Contributions
  • Membership in Professional Organization
  • Membership in Civic Organizations
  • Leadership
  • Other, Unspecified Outstanding Contributions to the Well Being of the Women of Okaloosa County.

Statements of Qualifications
Please submit in Resume Form:

  1. Nominee's professional history
  2. Nominee's awards and/or honors

* No more than 3 supporting documents on standard 8 x 11 paper (one-sided only) may be submitted if applicable. Specific awarded plaques may be listed; please do not photocopy.
A photo is required with the nomination form, preferably digital.

Biographical Information
Please submit a biographical narrative, double spaced and no more than 3 pages that includes the following information:

  1. Nominee's achievements and contributions (Supply dates where applicable).
  2. Nominee's accomplishments which have made a significant impact on her community, raised social conscience, established new codes of conduct, brought about greater self-determination of women, promoted diversity or served as the "first to accomplish something." PLEASE NOTE: This information may be used for publication, and quotations may be referenced from this statement.

It is not necessary to have made contributions in all of the above areas. No contribution is considered trivial or unimportant. Please mention ALL the contributions your nominee has made. From time to time there are extraordinary women who stand out in other areas, in which case the Commission will make a special award. If your Nominee is not selected this year, you may submit her name again next year.
